Medication

When it is absolutely necessary for the student to take a prescribed or over-the-counter medication during school hours, the nurse will cooperate with the family physician and the parents/guardians.

The parent/guardian will submit Doctor’s orders to the school nurse accompanied by the medication in the original prescription bottle. A form is available to be filled out by the prescribing physician. If your child’s medication is in a liquid form please request a second prescription to be left with the nurse at school. The prescription medicine MUST be delivered to the school nurse who will supervise the taking of the medication. No over-the-counter medications will be given without a medical doctor’s note. Call the nurse with any question or to request the necessary forms at 223-4404 ext. 14.

NO telephone request for medication from a parent/guardian or doctor can be accepted.

Children who are ill may NOT be sent home in a taxi unless accompanied by a parent/guardian or authorized adult other than the driver.

If a child is excused from gym for medical reasons, they may not participate in outdoor recess.
